Disability Care Report
We complete a comprehensive and holistic assessment of the client and detail their functional status at the time of assessment. We discuss their additional care needs with them and the primary caregiver. Relying on our own assessment and the evidence from medical and other healthcare professionals involved in the case, we quantify the past, current and future needs of the individual in terms of:

Additional Care
We evaluate past, present and future care requirements in respect of daily activities including personal care, meal preparation, domestic chores, transportation and home maintenance.
Equipment Needs
Our assessments identify the specific types of assistive equipment required to support an individual's daily functions and enhance their independence. This may include items like wheelchairs, kitchen aids, bath aids, or specialised beds.
Housing Requirements
We assess the need for modifications in the living environment to ensure it is safe, accessible, and conducive to the client's health. Recommendations can include ramps, stairlifts, widened doorways, or adapted kitchen and bathroom facilities to accommodate the individual’s physical limitations.
Therapy
We recommend additional therapeutic interventions, specifying the frequency and costs associated with occupational therapy and deferring to other relevant healthcare professionals, like physiotherapists, as necessary. Our goal is to aid in recovery, manage symptoms, and enhance overall independence through customised therapeutic strategies.
Transport
We assess the client's transportation needs to ensure they can travel safely to appointments, work, or community activities. Solutions may include adaptations to personal vehicles or provision of wheelchair-accessible vehicle.
Vocational/Functional Capacity
Our assessment consists of a thorough analysis of the client's functional capacity, examining their physical and cognitive abilities related to work. Based on this, we make recommendations about their suitability for employment.
Case Management
We provide ongoing case management to coordinate all aspects of a client's care. This includes regular assessments, coordinating with healthcare providers, adjusting care plans as needed, and ensuring that all services are effectively meeting the client’s evolving needs.
